Track your progress

Monitoring your blood sugar is an important part of your Xultophy® 100/3.6 treatment plan. These worksheets can help you stay on track with the diabetes care plan your health care provider recommends.

Blood Sugar Personal Progress Tracker

Step 1: Your health care provider completes a Dosing Worksheet

When taking Xultophy® 100/3.6, it’s important for your health care provider to give you your starting dose, your pre-breakfast blood sugar target range, and any adjustments you may need to make between appointments.

Take this Dosing Worksheet to your next appointment. Once you know your numbers, you’ll be ready to record your blood sugar levels with the Personal Progress Tracker and adjust as needed.

Step 2: Track your daily progress with a Personal Progress Tracker

Checking and tracking your blood sugar lets you and your health care provider know if your current treatment is working. Your health care provider will recommend when and how often to check it, as well as your target blood sugar range.

The Personal Progress Tracker is an easy way to log your daily blood sugar numbers, or FPG. Fill it out and bring it along to your next appointment with your health care provider so they can assess your progress.
